Services Description

Haven Health Autism Assessments provides mental health services including but not limited to:

• Autism assessments for teens and adults
• Psychiatric evaluations and medication management
• Integrative mental health care
• Treatment for trauma, PTSD, eating disorders, and related conditions
• Gender-affirming care
• Telehealth services

Not Emergency Services

Important: Haven Health Autism Assessments does not provide emergency or crisis services. If you are experiencing a mental health emergency, please:

• Call 911
• Go to your nearest emergency room

• Text HOME to 741741 (Crisis Text Line)

Patient Responsibilities

As a patient of Haven Health Autism Assessments, you agree to:

• Provide accurate and complete information about your health history
• Attend scheduled appointments or provide adequate notice for cancellations
• Follow the agreed-upon treatment plan
• Communicate openly and honestly with your provider
• Pay for services in accordance with our billing policies
• Treat our staff with respect and courtesy

Appointment and Cancellation Policy

We require at least 24 hours notice for appointment cancellations or rescheduling. Appointments cancelled with less than 24 hours notice may be subject to a cancellation fee. Repeated no-shows or late cancellations may result in discharge from our practice.

Payment Terms

Payment is due at the time of service unless other arrangements have been made. We accept various insurance plans and self-pay options. Please see our Payment & Insurance page for detailed information.

You are responsible for understanding your insurance benefits and any out-of-pocket costs. Unpaid balances may be sent to collections after 90 days.

Telehealth Services

Telehealth services are subject to additional terms and considerations, including:

• Services are only available to patients located in states where our providers are licensed
• You are responsible for ensuring a private, secure location for telehealth sessions
• A stable internet connection is required for video sessions
• Telehealth may not be appropriate for all conditions or situations
